According to this Tumblr Justin Bieber & Selena Gomez were spotted smoking Kama Sutra flavored hookah at Kaffe Rouge off Ventura Blvd in The Valley a couple of weeks ago. Kama Sutra is a book that provides guidance to sexual pleasure. Via AnythingDisney.

Hookah is just flavored tabacco (same difference as smoking a cigarette, except the smell is more bearable because it's flavored... and the way you smoke it is a lot more fun/classy). The flavor that Justin and Selena were supposedly smoking is supposed to "enhance sexual desires" (basically make you horny), although I'm sure it's more of you thinking that that's what it does, then that being what it actually does.
Most regular humans choose like fruit flavored hookah, so that's the only thing that makes this story a little questionable to me. (Although, I don't doubt that either one of them has smoked Hookah because they're both over 18. This person doesn't really have a reason to lie... and Selena looks a little "lightheaded" in that picture.) But this is all speculation.
If they did smoke Hookah though, trust me it's NOT that serious at all. They will both live. They're lungs will both be fine. Hookah isn't as available as a cigarette, so unless you smoke it every day because you own your own hookah pipe and flavored tabacco and coal blocks at home... (unlikely) it's the equivalent of simply "trying" a cigarette.

If you look at the caption on the photo of Selena with that guy it is tagged Kaffe Rouge. Then if you go read the tumblr story from a completely different person, she mentions that she was at Kaffe Rouge.
