Well Emma doesn't look sick, that's just her body type ... if you look at pictures of her not standing directly next to other girls she looks completely normal. Her face looks healthy and full, you don't even see her ribcage the way you do when people get unnaturally skinny.

Since making Spring Breakers, Vanessa has lost the weight she gained. That is why she looks so much slimmer in Berlin. I think Emma couldn't handle the lesbian scenes and Ashley was more suited for the role.
